Who do we accept referrals from?
We only accept referrals from statutory organisations, such as social services we do not accept self referrals.
What is the referral process?
· Contact LHP we will e-
· Upon receipt of completed referral form the application process begins.
· referral agent and applicant will be invited to view placement and for a Q&A session
· decision is made whether application is successful within 10 working days, once all relevant information to support application has been received.
· Terms and condition regarding support package is approved and move in date agreed.
LHP, aims to complete referral process within 15 working days.
We are also able to accept emergency placements. The application process for emergency placements will be fast tracked, please call us for further details 0208 532 8150.
Finance
Placements will primarily be funded by local authority and housing benefit.
